Work Hours

Hours are based on the needs of the individuals receiving services, not to exceed 8 hrs/day or 40hrs a week. Hours remain flexible to meet the needs of the individuals receiving services.

Position Qualifications

Experience

A Facilitator who posses the skills, training or education necessary to teach consumers to live independently.

Education

At least a High School Degree or GED and/or at least one year in a related field of human services.

Duties and Responsibilities

1. Rapid Communication.
2. Implement the program curriculum according to ISP/M.S.S. Assessment and IPP goals and objectives.
3. Provide direct 1:1 services to assist each consumer in obtaining his/her IPP and SLS objectives.
4. Monitor, prepare and maintain data regarding consumer's progress.
5. Insure consumer's safety.
6. Provide periodic feedback to the consumer, family member.
7. Participate in consumer's assessment, evaluation and planning, including attending IPP meetings.
8. Notify of any Special incident to supervisor Immediately.
9. Notify MSS administration of unexplained consecutive absences by the consumer.
10. Participate in monthly in-service/training.
11. Perform all related responsibilities as requested by the Area Manager.
